When I spotted this little side-twisted hairstyle on Khloe Kardashian yesterday, I immediately checked Jen Atkin's Twitter feed to see if she had posted anything on it. And, what do you know, she had already put the look up on her Tumblr...
Looks like a lock of her real hair was twisted and pinned back, and then a lock of faux hair was wrapped around it. And, no, I don't think it's faux just because she was out picking up hair extensions when she wore the style—it's the only way I can think of to get the twist that thick without pulling more of her real hair into it, which would have totally changed the vibe of the style.
